The purpose of this transfer format is to provide an exact and common format for the MORGAM Participating Centres (MPCs) to transfer baseline and repeat measurement data on the history of various new end-point diseases to the MORGAM Data Centre (MDC). Data in the format specified here should be submitted to the MDC for every member of each MORGAM cohort in which one or several of these diseses were followed up. The data should be sent through e-mail, not on paper forms. Data communication between the Participating Centres and the MDC.

Format specification

Columns of the format specification

ITEM NAME

name used for the item in the MDC.

SPECIFICATION AND CODES

specification and values of the variable. More details can be found in the section "Specific instructions for each item" below, or by following the hyperlink in column CHARACTERS.

CHARACTERS

specifies the format in which the value should be presented in the transfer data set. In the cases where the value of the variable is fixed, the value is also given in this column.

COLUMNS

specifies the position of each data item in the record.

General Instructions

Whenever these data are transferred to the MDC, a copy of the corresponding MONICA survey data (Form 20) should be included in the same transfer.

The record format is ASCII fixed length and record size is 40 characters. The position of each data item in the record is given in column "COLUMNS" of this data transfer format. Blank fields are not allowed in the record. There should be one record of data for every member of each MORGAM cohort.

This format corresponds with the layout of the information on the electronic data file which is transferred, and need not correspond with the format of locally used paper forms or electronic files.  To avoid errors, special attention should be paid in extracting these data items from the local data set.

Please contact the MDC for instructions if you cannot provide information as specified in this document or if you have any problems with the interpretation of the coding for any specific items.

Specific instructions for each item

Follow these instructions carefully when creating a computer file for the data transfer from the MPC to the MDC.

Number 32 indicates the Data transfer format: Baseline Data Addition 3.

Enter the version number from the heading of the "Data transfer format: Baseline Data Addition 3" which you are using. If the version number of the data transfer format which you are using is not "2", these instructions do not correspond to the format you are using. Check that you are using the valid version of the format.

Items CENTRE...SERIAL

These are key items used for merging the different records of the same individual. Please make sure that the codes are identical with those used for the same person in the same examination round in "Data transfer format: MONICA survey data".

Code 88 (irrelevant). The purpose of this item is to reserve fixed columns for the key items in all data transfer formats. This is a key item in the event data formats, but irrelevant for the other data transfer formats.

The first two columns are for the month; code 01 - 12, or code 99 if the month is not known. Enter the year of birth, in full four characters, in the last four columns. If the year of birth is not known, which should be very rare, derive the year of birth from an estimate of the age.

This item is the same as item MBIRTH of the "Data transfer format: MONICA survey data", and it will be used for double checking the key items. If you compile the data for this record from various sources, please extract the data for this item from a different source than for the MONICA survey record.

Items HIST1DM1...HIST2DM2: History of type 1 and type 2 diabetes mellitus

These items complement the information provided in item HISDIAB of Form 21, which does not specify the type of diabetes. These new items became relevant when the follow-up for type II diabetes was included in MORGAM.

Code 1 (yes) if the presence of type I diabetes mellitus was based on a review of medical records or if information of type I diabetes mellitus was extracted from drug reimbursement register, from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of type I diabetes mellitus.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included either of the questions 'Have you ever been told by a doctor or other health care professional that you have type I diabetes?' or 'Do you have (suffer from) type I diabetes?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if such a question was included in baseline questionnaire and the answer was negative.
Code 9 (insufficient data) if such a question was not included in the baseline questionnaire or, if included, it was not answered.

Code 1 (yes) if the presence of type II diabetes mellitus was based on a review of medical records or if information of type II diabetes mellitus was extracted from drug reimbursement register, from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of type II diabetes mellitus.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Items HISHF1...HISHF2: History of heart failure

Code 1 (yes) if the presence of heart failure was based on a review of medical records or if information of heart failure was extracted from drug reimbursement register, from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of heart failure.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included the question 'Have you ever been told by a doctor that you have had heard failure?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if baseline questionnaire included such questions but the answer was negative.
Code 9 (insufficient data) if no such question was included in the baseline questionnaire, or it was included but the answer is unknown.

Items HISAF1...HISAF2: History of atrial fibrillation

Code 1 (yes) if the presence of atrial fibrillation was based on a review of medical records or if information on atrial fibrillation was extracted from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of atrial fibrillation.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included the question 'Have you ever been told by a doctor that you have had atrial fibrillation?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if baseline questionnaire included such questions but the answer was negative.
Code 9 (insufficient data) if no such question was included in the baseline questionnaire, or it was included but the answer is unknown.

Items HISPVD1...HISPVD2: History of peripheral vascular disease

Code 1 (yes) if the presence of peripheral vascular disease was based on a review of medical records or if information on peripheral vascular disease was extracted from drug reimbursement register, from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of peripheral vascular disease.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included the question 'Have you ever been told by a doctor that you have had peripheral vascular disease?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if baseline questionnaire included such questions but the answer was negative.
Code 9 (insufficient data) if no such question was included in the baseline questionnaire, or it was included but the answer is unknown.

Items HISVTE1...HISVTE2: History of venous thromboembolism

Code 1 (yes) if the presence of venous thromboembolism was based on a review of medical records or if information on venous thromboembolism was extracted from hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of venous thromboembolism.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included the question 'Have you ever been told by a doctor that you have had venous thromboembolism?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if baseline questionnaire included such questions but the answer was negative.
Code 9 (insufficient data) if no such question was included in the baseline questionnaire, or it was included but the answer is unknown.

Items HISCAN1...HISCAN2: History of cancer

Code 1 (yes) if the presence of cancer was based on a review of medical records or if information on cancer was extracted from cancer register, drug reimbursement register, hospital discharge register or other health information system.
Code 2 (no) if source of information did not verify the presence of cancer.
Code 9 (insufficient data) if no such source of information was used or if information of such assessment is not available.

Code 1 (yes) if the baseline questionnaire included the question 'Have you ever been told by a doctor that you have had cancer?' (or a similar question) and the answer was in the affirmative.
Code 2 (no) if baseline questionnaire included such questions but the answer was negative.
Code 9 (insufficient data) if no such question was included in the baseline questionnaire, or it was included but the answer is unknown.

ROUNDS indicates the examination round and therefore separates the baseline measurement and possible repeat measurements. Together with items CENTRE...EVENT, ROUNDS is a key item. Please make sure that the code is identical with that used for the same person in the same examination round in the "Data transfer format: MONICA survey data".

ROUNDS is located exceptionally at the end of the data transfer format because repeat measurements were introduced to MORGAM at a later stage. For the data transferred earlier, item ROUNDS was coded retrospectively in the MDC.

Please do not hesitate to contact the MDC if you have any uncertainty in the coding of item ROUNDS.
