"Selected to the original sample" - MDC should have a record of each of these persons in the format of form 05 (Survey serial number inventory format).

"Selected to the sample but found not eligible" - these are persons with code 3 for STATUS in form 05 (Survey serial number inventory format).

"Selected to the sample and found eligible" - MDC should have a record of each of these persons either in the format of form 04 (Core data transfer format - survey data) or in the format of form 08 (Data transfer format - survey non-responder data). The number in this row should be equal to number of included to the original sample minus number of classified as non-eligible.

"Participated in the survey" - MDC should have a record of each of these persons in the format of form 04 (Core data transfer format - survey data).
